4k Screenings for 2013-2014

CHILD DEVELOPMENT (4K) PROGRAM

 

Individual screenings will be conducted

May 20-24, 2013

by appointment only

 

TO MAKE AN APPOINTMENT, PLEASE CALL theelementary school that serves children in the area where you live.

 

When: April 29 – May 3, 2013

Time:Between 8:30 a.m. and 3:00 p.m.

 

·Woodland Heights Elementary – 576-0506

·Jesse Bobo Elementary – 576-2085

·Arcadia Elementary– 576-1371

·Child Development Center – 576-4886 (for children who live in the Lone Oak Elementary and FairforestElementary attendance areas)

·Anderson Mill Elementary – 576-6539 (for children who live in the Pauline Elementary, Roebuck Elementary, and Anderson Mill Elementary attendance areas)

·West View Elementary—576-1833 (for children who live in the West View attendance area)

 

Bring to your MAY Appointment:

1) Your child, who will be fouryears old on or before September 1, 2013

2) Your child's certified birth certificate

3) Your child's South Carolina Certificate of Immunization

4) Proof of residence (lease,deed to property, property tax receipt, realtor  

    contract, rental receipt)          

5) Proof of any services received from DSS (TANF, SSI, Medicaid)

6) Proof of enrollment/involvement in Family Literacy Program

(Even Start, PAT, PCHP)

 

No child will be screened without a birth certificate and a proof of residence.
