CHILD DEVELOPMENT (4K) PROGRAM

 

Individual screenings will be conducted

May 14-18, 2012

by appointment only

 

TO MAKE AN APPOINTMENT, PLEASE CALL the elementary school that serves children in the area where you live.

 

When:April 23 – April 27, 2012

Time:Between 8:30 a.m. and 3:00 p.m.

 

·Woodland Heights Elementary – 576-0506

·Jesse Bobo Elementary – 576-2085

·Arcadia Elementary– 576-1371

·Child Development Center – 576-4886 (for children who live in the Lone Oak Elementary and FairforestElementary attendance areas)

·Anderson Mill Elementary – 576-6539 (for children who live in the Pauline Elementary, RoebuckElementary, West View Elementaryand Anderson Mill Elementaryattendance areas)

 

Bring to your MAY Appointment:

1) Your child, who will be four years old on or beforeSeptember 1, 2012

2) Your child's certified birth certificate

3) Your child's SouthCarolina Certificate of Immunization

4) Proof of residence (lease, deed to property, property taxreceipt, realtor  

    contract, rentalreceipt)          

5) Proof of any services received from DSS (TANF, SSI,Medicaid)

6) Proof of enrollment/involvement in Family Literacy Program

    (Even Start, PAT, PCHP)

No child will be screened without a birth certificate and a proof of residence.

4) Una prueba de residencia(un recibo de la renta/alquiler, un titular de             casa, etc)

6) Una prueba de actividad enel programa de Family Literacy
